The Access & Support program advocates for people who due to diverse needs or barriers, have been unable to access services to maintain independent living.
If you are in need of support to stay independent in your home and community, our Access & Support program can link you with essential support services.

We support you to stay independent and safe
Accessing support services can enable to you live safely and independently in your own home and community.
But if you’re experiencing barriers to access supports, our Access & Support program can link you to the services you need.
The Access & Support program supports people who are:
- financially and/or socially disadvantaged
- living with dementia or cognitive impairment
- a veteran
- living with an illness or disability
- homeless or at risk of homelessness
- a care leaver
- living in a rural or remote area
- culturally or linguistically diverse
- identify as LGBTIQ+
- Aboriginal or Torres Strait Islander
Access help to find and apply for services that suit your needs
If you are aged 65 years or over, an Access & Support Worker can:
- link you into services to help enable safe and independent living in your home
- provide information about the My Aged Care process, and assist you to register for this
- support you to apply for help around your home through My Aged Care
- assist you with completing paperwork and/or online applications
- visit you in your home to assess your needs
- refer you to services, support you at appointments, and help you to understand service fees and costs (if applicable)
- support you to access government payments and services (e.g. Centrelink)
- liaise with service providers to ensure that your needs are being met
If you are under 65 years of age, an Access & Support Worker can:
- link you with services that are available to keep you living in your home as safely and independently as possible
- provide information about Living at Home Assessments and/or the NDIS process
- support you to gather evidence for NDIS and Centrelink claims
- assist you with completing paperwork and/or online applications
- visit you in your home to assess your needs
- refer you to services, assist you to get help around your home, and help you to understand service fees and costs (if applicable)
- liaise with service providers to ensure that your needs are being met
How do I access this service?
Individuals, carers, and family members living in the City of Ballarat, Moorabool Shire, Hepburn Shire and Golden Plains Shire can access this service by calling our Ballarat office.
Community service organisations and health professionals can also make referrals into the Access & Support program by calling us.
Fees
There is no charge to access this service.
This service is funded by the Victorian State Government Department of Health.
